Communications and relationships: Recording and reporting
Reporting and recording are fundamental to ensuring good outcomes for people who you care for and support. This badge will help you to provide evidence that you know, understand and can apply the principles of recording and reporting that are at the core of best practice. You will need to read the Recording and reporting chapter of the Communications and Relationships ebook before you can apply for this badge.
To get this badge you must:
- complete the activity in the Recording and reporting chapter and attach your note to your evidence for this badge
- think about situations in which you have encountered barriers to communication with people with whom you work
- using the information in this chapter, write a short reflective account of a minimum of 150 words to explain why it is important that you must record and report information
- write a short reflective account of a minimum of 100 words to explain how you will use the knowledge you gained from this chapter in your practice in future.
We will return applications that do not provide enough evidence showing how you have met these criteria.
